a | Qualification | Minimum qualification at least metric preferably with Science obtaining 40% marks. |
b | Age | 15 to 40 years. |
c | Experience | Experience of working in the community will be preferred. |
At the end of 10 weeks after starting the classes there will be an Internal examination. Students declared Internal Examination unsuccessful will be re-examined alter 4 weeks, if unable to clear the second attempt; the student will be expelled from the training programme.
Last date for submission of Admission forms Is 15-05-2019 however the form can be accepted at the time of Last Date .
Interview under special circumstances.
Attested Photocopies of following documents should be attached with application form & bring original documents at the time of interview.
a) Matric Certificate from B.I.S.E showing date of birth.
b) Matric Marks Sheet.
Documents
C) Domicile / PRC.
d) C.N.I.C
e) 4 Passport Size Photographs duly attested.
1) Resident Certificate from Union Council.
Stipend | The candidates will be paid stipend Rs. 5,000/- per month during training. | |
Residential Accommodation & Mess | Hostel Accommodation and mess arrangement are available. | |
Application form | Application are to be submitted on prescribed form which can be had from the office of Principal of all CMWs School I Nursing & Midwifery Schools / Public Health Schools free of cost. |
