How to Apply
an Application Form for Allied Health Sciences program shall be filled online, Login at www.numspak.edu.pk and click the link Apply in AHS Programs” and fill the application form, Candidates are required to scan and attach following attested copies of mandatory documents with Online application form.
(1) SSC Certificate.
(2) HSSC Certificate.
(3) Candidate’s CNIC.
(4) Father! Guardians CNIC.
b. After successful attachment of scanned copies then click the Submit” button.
Upon successful submission of the application, the candidate will be able to print Application Processing Fee Challan.
d. Candidates are required to visit the nearest branch of Habib Bank Ltd (countrywide) to deposit application processing fee @ Rs 30001- latest by Friday 08 November, 201 9.
e. After depositing fee, scan the bank stamped ‘Paid Fee Challan” and attach online with the application form, then click the “Final Submit”

a. Admit Card will be available on the NUMS website from 20 November 2019.
b. Details of the address of the Entry Test Center will be mentioned on candidatesAdmitting Card.
6. Syllabus for Entry Test
The syllabus of the entry test is available on NUMS ‘Q.bsite https://numspak.edu.pk/ahs/registration
7. The pattern for EntryTest 7’
a. Entry Test shall comprise a single question paper containing 100 multiple-choice questions (MCQs) divided into the following four (04) sections for FSc (Pre-Medical) and (05) sections for FSc (MLT):
there shall be No Negative Marking in the test, the duration of the test shall be 120 minutes (2 Hours).
Important Instructions for students:
a. The candidates must reach their Entry Test Center at-least one hour before the commencement of the test on Sunday 24” November 2019 (i.e. by 09:00 AM). The center will be sealed? closed at 09:30 AM. The entry Test start time is 10:00 AM
b. The candidates must bring following with them on the test day:
(1) Print out of Admit Card.
(2) Original Passport? CNICI NICOP? POC? Smart card for Juvenile or Original NADRA token slip? Form ‘B’ with any Valid Government document having a photograph.
(3) A clean, hard Clip Board without any marks or written material.
(4) Two good quality Blue? Black ballpoint pens (Not Pencils, Pointers or Markers).
c. Hostel facilities are not available.
Note The candidate who fails to bring Original CNIC/ NICOP! POC/ Passport, smart card for Juvenile and print-out of his! her Admit Card shall not be allowed to appear in the test.
